Max Life Insurance, one of India’s leading life insurance companies, highlights the claims settlement process of Max Life Pradhan Mantri Jeevan Jyoti Bima Yojana- a recently launched scheme by the government. The risk cover will be provided to the people in the age bracket of 18 years – 50 years and the claim amount of Rs.2,00,000 is payable on death of a customer to his / her nominee(s). Max Life Insurance has always focused on customer centric approach and provided all type of assistance to its customers. This is also another step to inform and assist customers on the hassle free claim settlement process and to ease their lives. Nominee can refer to the following steps while applying for the claim.
- Nominee to contact concerned branch of the Bank where insured member had his savings bank account & was covered under Max Life Pradhan Mantri Jeevan Jyoti Bima Yojana
- Nominee will have to submit duly completed Claim Form, original / attested copy of Death Certificate of insured member along with copy of cancelled cheque of nominee’s bank account or nominee’s bank passbook where name & account number is printed
- NEFT mandate form attested by Bank can be submitted in case nominee’s bank passbook or cancelled cheque with printed name & account number are not available
Documents to be submitted by the nominee:
- Duly filled Claim Form (to be completed by Nominee & Bank)
- Original/Attested copy of Death Certificate issued by Municipal Authorities.
- NEFT Mandate form/Copy of Bank passbook/Cancelled Cheque where name & account number of nominee are printed
- Attending Physician Statement (Form C) /Hospital records for members who have given health declaration at enrolment stage
Additional documents in case of accidental death:
- First Information Report (FIR).
- Post Mortem/Viscera Report and Panchnama
- Final Police Investigation Report
Once the nominee has submitted all the documents to the bank, it will verify claims form and fill relevant columns. The Bank shall also conduct a quality check on the claim documents to verify member and nominee details before sending them to Max Life Insurance at Operations Center - 2nd Floor, 90 A Udyog Vihar Phase – IV, Sector 18, Gurgaon 122015.
