TCC Case Studies to Guide Community based Tobacco Control Project in Delhi

New Delhi, Delhi, India, Tuesday, June 01, 2010 -- (Business Wire India) -- Smt. Sheila Dikshit, Chief Minister, Delhi, and Prof Kiran Walia, Minister of State, Health & Family Welfare, launched the "Tobacco Cessation Clinics Case Studies" of successful public and private sector interventions on World No Tobacco Day. The case studies document operating models of successful Tobacco Cessation Clinics (TCCs) from across the country which serve as a reference guide for establishing TCCs.

"This year our objective is to create a successful and replicable intervention model for creating awareness on tobacco cessation and smoking cessation clinics through referrals and community linkages. At the same time, we hope to strengthen treatment compliance in existing tobacco cessation clinics with the support of doctors, NGOs, families at the institutional and community levels", stated Prof. Kiran Walia.

A number of TCCs have been established across the country. They provide three levels of support that include awareness on the harmful effects of tobacco use, counseling and pharmacotherapy.

These case studies document some of the initial experiences from both public and private hospitals as well as individual clinics including Max Super Specialty Hospital, Delhi, IHBAS, Delhi, NIMHANS, Bangalore, Empower Wellness, Mumbai and the Regional Cancer Hospital, Cuttak. "These case studies will serve as a practical guide for setting up new tobacco control clinics in India and in other countries to take the War on Tobacco to the next level, "said Mr. Kewal Handa, Managing Director, Pfizer Limited.

The Tobacco Cessation Clinicians Council of India has developed a TCC Algorithm that has been derived from the practical experiences of the clinics documented in this study. The Algorithm provides a comprehensive guide towards managing a tobacco cessation clinic (TCC) that includes referrals, counseling, self-help tips and pharmacotherapy, if required. "The algorithm was developed in partnership with Pfizer to provide a framework to involve primary care doctors in tobacco cessation and to bridge the gap between tertiary care specialty institutions and primary care clinics", said Dr. Nimesh G. Desai, Director, IHBAS and member of the Tobacco Cessation Clinicians Council. Tobacco Cessation Clinicians Council (TCCC) was established with the goal of ensuring the involvement and participation of medical doctors at all levels, and specifically the primary care doctors, in tobacco cessation.

"Certain areas of the brain undergo chemical changes as a result of extended use of various forms of tobacco. That makes the individual consume nicotine repeatedly. Will power alone is not always sufficient enough to stop tobacco consumption. Professional help would be essential to address nicotine addiction," said Dr Desai. Another challenge is the low level of compliance from tobacco users that opt for clinical interventions. As a result, the number of successful quitters remains low with several cases of relapse into tobacco addiction

During the event, Prof Kiran Walia launched an integrated pilot project on Community based interventions for Tobacco Control in East Delhi and South Delhi. The clinical case studies will help establish an integrated community led pilot project with the involvement of families, physicians and institutions to develop a holistic approach to tobacco control through treatment, care and support. "By targeting smokers at the community and institutional levels, the project will close the circle through counseling, referrals, and patient follow ups. These efforts will lead to increased treatment compliance, reduced relapse rates and successful quitters", said Dr Ratna Devi, CEO, Chronic Care Foundation.

The pilot project is an attempt to address these challenges through a holistic two pronged approach to:

1. Provide information to smokers, households, and communities on tobacco cessation support available through government and other stakeholders through referrals to existing Tobacco Control Clinics (TCCs) in East and South Delhi.

2. Support treatment compliance in these TCCs by providing OPD and Community level counseling and follow up services to help smokers successfully quit.

The Delhi Government will implement the project through the Institute of Human Behavior and Allied Sciences (IHBAS), in partnership with Chronic Care Foundation (CCF), Pfizer Limited, Delhi Medical Association and public/private hospitals and clinics in Delhi.

Chronic Care Foundation (CCF)

CCF is a national level NGO set up as a not for profit foundation to address issues relating to chronic disease. CCF is committed towards improving the care and outcome of patients with Chronic Non Communicable Diseases (NCCD) like cardiovascular diseases, diabetes and chronic kidney diseases. The high and increasing incidence of cardiovascular disease, diabetes, and renal failure has led to the setting up of the Chronic Care Foundation with stakeholders, aimed at preventive healthcare for chronic diseases and better management of the same. The Foundation's activities range from conducting studies to identify the cause of these diseases to developing programs for promoting healthy behavior and working with medical practitioners for better delivery and outcomes.

Chronic Care Foundation has carried out awareness generation on Tobacco as a risk factor for chronic disease in coordination with its partner NGOs in schools, slums, general community level programme in Resident Welfare Associations. It has carried out capacity building of its partner NGOs to work on tobacco control programme and created a trained team of street theatre groups and magicians for community level awareness generation activities. Chronic Care Foundation in partnership with Pfizer Limited launched a mass awareness and education campaign titled "Nasha Mukht Punjab" - (Punjab da josh - Punjab di Shaan) focusing on tobacco cessation and awareness generation on chronic disease in different colleges and community places in Amritsar of Punjab

Institute of Human Behavior and Allied Sciences (IBHAS)

Institute of Human Behaviour & Allied Sciences (IHBAS) is an autonomous body funded jointly by Ministry of Health and Family Welfare, Government of India and Government of NCT of Delhi. The principal objectives of the Institute are providing super specialty care in psychiatry, neurology and behavioral sciences, conducting research in the field of psychiatry, neurology & behavioral sciences and providing comprehensive training for MD, DM, M. Phil. & Ph.D. level in psychiatry, neurology & behavioral sciences as well as research activities.

IHBAS has a state of the art Tobacco Cessation Clinic which is part of a National Tobacco Control Initiative by the Ministry of Health and Family Welfare, Government of India and the World Health Organization. The Institute is recognized as the National Coordinating and Resource Center for Tobacco Control. This clinic is held thrice a week on every Monday, Wednesday and Friday afternoon under the care of a multi-disciplinary team. The treatment modalities offered are both pharmacological and psychosocial with emphasis on counseling and behavior management.

Pfizer India has initiated several innovative projects to help build India's healthcare infrastructure. In 2009, Pfizer launched an ambitious project to establish 600 smoking cessation clinics across the country to provide a comprehensive approach to help reduce tobacco consumption. Pfizer has partnered with Dr. Nimesh Desai from IHBAS to develop a Smoking Cessation Algorithm that provides a comprehensive reference guide towards establishing and operating a tobacco cessation clinic (TCC). The TCC algorithm has been adopted by the Tobacco Cessation Clinicians of India and a number of public-private TCCs have been modeled on the TCC algorithm.

In March 2010, Pfizer partnered with the RML Hospital and Chronic Care Foundation to spearhead a campaign to reduce the burden of tobacco consumption in India by engaging key stakeholders to initiate a War on Tobacco. The program is being rolled out through a series of workshops and training activities in partnership with multiple stakeholders. The goal is to get the media to lead a new campaign to introduce clinical approaches to quit smoking and showcase successful tobacco cessation clinics; NGOs to take on an active role to conduct referrals and provide basic counseling to address the psycho-social aspects of smoking and doctors to start tobacco control clinics in their individual/hospital practices.
